Two players finish a game other than the last game of the match close to the expiry of the time limit and shuffle up for the next game. What stage of proceedings must they have reached before time being called to receive the five extra turns?

(My opinion/understanding is that both players must have declared they have kept their hands, or taken some other affirmative equivalent action.)

From section 2.3 on Pregame Procedures in the MTR:

The game is considered to have begun once all players have completed their mulligans.

So yes, in order for the game to start, mulligans must be resolved and players need to keep their opening hands.

This is an extract from MTR, section 2.3:

*The game is considered to have begun once all players have completed their
mulligans. Pregame procedures may be performed before time for the match
has officially begun.*

But in section 2.5 from the MTR, End-of-Match procedure, it says nothing
about when we should allow players to play the 5 extra turns. I've never
allowed a player to begin a game once time is called because that's what
I've learned from other judges.

Until the game has begun - as defined in MTR 2.3 - we don't apply the five extra turns rule.
