Failure to properly resolve Brainstorm

Hi all! I was just on Twitch watching a stream and the following situation came up in chat. Some of you may see that this is similar to a situation that recently happened in a pro-level event, and this was inspired by that situation (not going to hide it).

Player A casts Brainstorm. He draws 3 cards, but only puts back 1. Play continues, and later a judge is called by player B who notices that Player A has too many cards. Is this GRV for failure to properly resolve Brainstorm, or is it DEC for having extra cards in hand that can't be accounted for?

Related question, which was also brought up in the chat:

Player A casts Hymn to Tourach. Player B only discards 1 card (somehow). Same situation, turns later, judge is called. Is this DEC or GRV?

If the players can work out the cause of the extra card, then it's been accounted for. See this related question.

Both sound like GRVs to me. We even have a partial fix for exactly this
situation, and it's an example of GRV:

Example:
E. A player casts Brainstorm and forgets to put two cards back on top of
his library.

Partial fix:
If a player forgot to draw cards, discard cards, or return cards from their
hand to another zone, that player
does so.
